All you need is a really good graphics program. Most likely the person who did that layout was using a professional package like photoshop or paint shop pro. They basically create a large scale picture that is the size of a desktop and then they slice it. They make it into different sized page segments that can later be put together again using HTML and Cascading stylesheets (CSS).

The only cost you have to have is to buy the graphics program. Otherwise if you can find one for free, then you have it made.
